Parents Must Bring The Following:
1.) Child for Registration
2.) Official Copy Of Child’s Birth Certificate
3.) Child’s Social Security Card
4.) Proof Of Residence (Acceptable Forms Include: Utility Bill, Home Insurance Policy, Rental/Lease Agreement, And Most Recent Income Tax Return Or Property Statement

What Must Be Provided By First Day Of School:
1.) Child’s Immunization Record (Form 3231)
2.) Child’s Vision, Hearing, Dental, And Nutrition Screening Form (Form 3300)
